Your name exactly as it appears on your birth certificate.
If you are adopted it would be your pre-adoption birth certificate.
Your date of birth, and the name you now use to introduce yourself
To order reports:-
Send an e-mail with the above details and specify the reports that you would like.
I will send bank details and confirmation of cost to you and your reports will be e-mailed to you within 24 hours of receipt of proof of payment, which can be made be direct deposit or eft.
